The Accountable Health Communities Opportunity: Jumpstart Your Social Needs Strategy Web Conference
Through the AHC program, CMMI will test the extent to which connecting patients to social resources improves health care outcomes and reduces costs. AHC grants will be awarded to up to 44 geographically dispersed providers in amounts ranging from $1 million to $4.5 million over 5 years. AHC program applications are due by March 31st.
